I authorize The University of Alabama to release my academic record each term to my high school. This release is counter signed by my parent or legal guardian, if I am less than 18 years of age. This release shall remain in effect until I provide written notice to the UA Early College office to discontinue the release. I understand that I am subject to the Federal Education Rights and Privacy Act of 1974 (FERPA).

I decline the offer of academic advising made available to me by my academic advisor in the UA Early College program. I understand that this may be to my disadvantage in the future and that I am wholly responsible for any and all decisions about my registration I might make for this upcoming academic term. I understand that this waiver applies only to the current advising session/upcoming academic term at The University of Alabama and that I will have the opportunity for advising during the next advising period and for future academic terms at The University of Alabama.

I also understand that:

1. I will be required to submit a course request upon completion of this academic advising waiver.

2. My course request may be denied and academic advising may be required by UA Early College staff.

3. I have the option of participating in academic advising during the current advising period should I have follow-up questions or need assistance concerning future plans.

NOTE: Submitting this form indicates that the student and parent/guardian have waived the opportunity for advising/scheduling assistance from a professional advisor of the UA Early College program during his/her current advising period at The University of Alabama. The student is held solely responsible for course placement, prerequisites, and other aspects of enrollment.
